Issuance of Disaster Victim Certificates for Residential Buildings/住家の「り災証明書」の発行について

About Disaster Victim Certificates forResidential Buildings

 

If your residence sustained damage due to the heavy rainfallbeginning on August 8, 2025, the City will conduct a damage assessment surveyof the affected residence upon application by the victim. Based on the surveyresults, the City will issue a Disaster Victim Certificate.

 

Damage classifications for residencesinclude the following:

・「Total destruction」

・「Large-scale partial destruction」

・「Medium-scale partial destruction」

・「Partial destruction」

・「Minor partial destruction」

・「Minor partial destruction or less(partial damage)」

・「Above-floor flooding」

・「Below-floor flooding」

 ※Depending on the survey results, it ispossible that the determination will be “no damage.”

 

Eligible Applicants

・Those whose “residence” has been damaged

※Applications may be submitted by the headof the household living in the residence at the time of the disaster,regardless of ownership.
・Management associations, etc., of sectional ownershipbuildings (condominiums) whose common areas have sustained damage

※Carports, storage sheds, gates, andsimilar structures are not eligible.

How to Apply

 ・Apply in person at the counter or via theMyNa Portal (online application). Click here for MyNa Portal link (externalsite).

 ・The City will conduct a damage assessmentsurvey to confirm the extent of damage.
 ・A damage assessment survey will be conducted, and thecertificate will be issued later at the application counter.

 

※If you are unable to visit the counter dueto evacuation to a distant location, please contact us.

 

※However, if the damage is minor and asurvey is not required, but you wish to obtain a Disaster Victim Certificatefor “partial damage,” you may submit photographs or other documentation showingthe damage. Upon confirmation, the certificate will be issued. For applicationsmade in person at the counter, same-day issuance is possible.

 Once contacted, please collect yourcertificate at a counter listed below.

 ※Depending on the documents brought andthe results of the confirmation at the counter, same-day issuance may not bepossible.

Required Documents

 1 Identificationof the person receiving the certificate (driver’s license, My Number Card,etc.)

※If a proxy is receiving the certificate,identification for the proxy is required.

 2 If the addresson your Residence Certificate differs from the disaster address, documentsproving it was your principal residence (utility bills, etc.)

※To objectively verify your principalresidence, additional supporting documents may be required and assessed on acase-by-case basis. Please consult the Welfare Section of your ward (refer tothe phone numbers above) before visiting.

 3 Letter of Proxy(required if the recipient is not the applicant or a member of the samehousehold)

 4 Application form

5  Management regulationsand minutes of general meetings, etc. (for management associations of sectionalownership buildings whose common areas have sustained damage)

  ※Documents indicating the name of themanagement association and the name of its representative

     

  ※However, if the damage is minor and adamage assessment survey is not required, but you wish to obtain a DisasterVictim Certificate for partial damage, you may submit photographs or otherdocumentation showing the damage. Upon confirmation, the certificate forpartial damage will be issued. For applications made in person at the counter,same-day issuance is possible.
